WHAT ARE DENTAL IMPLANTS AND WHY SHOULD YOU GET THEM?

Dental Implants are replacements for tooth roots and missing or damaged tooth. The procedure replaces damaged or missing tooth with metal (Titanium) screw-like posts and artificial ceramic teeth that look and function like any normal tooth.

The implants could be done for one tooth to several teeth or the entire teeth set. Dental implants are better alternatives to conventional techniques such as ‘bridges’ or ‘dentures’. Due to the great extent of relief offered by this procedure and this being very similar to natural teeth, they are seen as a welcome technique desired by many.

DENTAL IMPLANTS VS BRIDGE

For a dental bridge, the adjacent teeth to the missing teeth/tooth need  to be prepared and reduced in height and width to accommodate the prostheses.If you develop a deep cavity, infection or periodontal (gum) disease with either supporting tooth, then you may lose the entire bridge.

With a dental implant, the crown is directly connected to the implant, and you have no space between the crown and the gum line. Unfortunately, with bridges — depending on how much space is available and the type of food you eat — you'll end up getting food lodged underneath and between the fake teeth. 

A dental bridge may need to be replaced every 5 to 15 years, depending on proper care, while implants are considered a more permanent solution.

SINGLE TOOTH DENTAL IMPLANTS

When a tooth is missing, a replacement tooth should be put into the open space in order to maintain proper dental alignment and health. For years patients were advised to set their desires aside and accept the limitations of a FPD(bridge).However the most natural method to replace a tooth is to use an implant, rather than preparing adjacent teeth and joining them together with a prostheses.

If you are missing a single tooth, one implant and a crown can replace it. A dental implant replaces both the lost natural tooth and its root without impairing gum health or damaging surrounding teeth.

  • Maintains bone in site and prevents its further deterioration.
  • Restore the function of natural teeth .
  • No adjacent teeth need to be cut down for an implant
  • Easier to keep clean than a bridge. The gums can recede around a bridge, leaving a visible defect when the metal base or collar of the bridge is exposed. The resorbed bone beneath the bridge can lead to an unattractive smile.

The cement holding the bridge in place can wash out allowing the bacteria to decay the teeth which anchors the bridge.

Dental implants can be used to stabilize your denture, preventing it from moving and giving you the comfort of a perfect fit.

Wearing a conventional full denture can be uncomfortable as they can be restrictive in terms of speech and eating. Issues regarding gagging, taste, speech and instability are also often complained about.

Dentures and bone loss

After long-term denture wear, the underlying bone recedes .The more often a patient wears a denture, the greater the bone loss. Yet 80% of denture patients wear the dentures day and night, which accelerate the bone loss and reduce the chances of successful future dental implant surgeries.

As the bone recedes, the denture starts developing sore spots and start hurting the patient on eating .They also become loose over a period of time, all this makes denture wearing uncomfortable.

A more retentive treatment option would be one using dental implants to support the denture.

In this, the denture is removable by the patient, or fixed and only removable by a dentist. The significant advantages to these treatment options are that the dentures are much more stable and provide a more natural function. Chewing becomes easier and bone levels are maintained, providing long-term stability and maintained face shape. Also, with dental implant-supported dentures it is not necessary to have material covering the majority of the palate as retention is gained mechanically from the dental implants, so taste is unaffected and gagging is reduced if not completely eliminated. Furthermore, as bone height is maintained, the face shape does not change and denture adjustments are far less likely to be required, leaving you with long-term, comfortable dentures.

Denture implants options

Following options are available for denture implant-retained dentures

Locators: In this 2 to 4 implants are placed in the jaw. The denture is click fitted over the implants and can be removed by the patient with ease. This works much like click button system and is often the desirable option when patients like to minimize the feeling of prosthesis in their mouth.

Ball attachment: Similar to locators, minimum 2 implants are placed in the jaw .The denture is fitted with the help of ball attachments over the implants.

Milled-bar: In this implants are splinted with a bar and denture is fitted on top of it.

Implant supported bridges

Implants can be used to replace more than one missing tooth using two or more implants and splinting them in the form of an implant supported bridge.

When the patient is missing all their teeth in one or both jaws, procedures like ALL ON 6 /ALL ON 4 are used to replace the missing teeth.

This procedure involves placement of 6 implants in one jaw without the need of surgical exposure for bone grafting. An immediate provisional restoration in given the same day .Once the implants are integrated after 4-6 months, the patient is called back for final permanent restoration .

All-on-4® Treatment Concept

All on 4 is a treatment procedure that essentially involves replacement of all missing teeth in one jaw by using only 4 implants. The implants are placed in strategic areas of the jaw where there is more available jaw bone often eliminating the need for costly and time consuming bone grafting, sinus lift procedures.

The obvious advantage of this procedure is that the patient goes home with fixed provisional teeth on the same day.

In patients with insufficient bone, grafting can be done to improve the quantity and quality of the bone to place the implant.

Bone loss may be because of the following reasons:

  • Long term missing tooth/teeth
  • Periodontal diseases
  • Trauma due to sports or accidents

The graft used may be taken from different part of your own body ( autologous) or from cadaveric bone obtained from bone banks (allograft) and from synthetic sources (alloplasts).After bone grafting ,the implants may or may not be placed in the same sitting.

The patients are recommended to wait for a period of 4-6 months before final teeth are given.

Collagen membrane and PRF (platelet rich fibrin) are often used with grafting to hasten the healing and achieve more favorable results.PRF is obtained from the patient’s own blood and is used to accelerate the healing of soft and hard tissues. There is no risk of an allergic reaction as its patients own blood that is used to make PRF.

SINUS LIFT PROCEDURE

Sinus lift surgery is done in the upper jaws when there is insufficient bone in the premolar and molar region for the placement of dental implants.

It is done by lifting the membrane of the maxillary sinus followed by placement of bone graft.

Depending upon the amount of bone available, two types of sinus lift procedures exist-

  • Indirect sinus lift
  • Direct sinus lift

Indirect Sinus Lift

SINUS LIFT PROCEDURE

Sinus lift surgery is done in the upper jaws when there is insufficient bone in the premolar and molar region for the placement of dental implants.

It is done by lifting the membrane of the maxillary sinus followed by placement of bone graft.

Depending upon the amount of bone available, two types of sinus lift procedures exist-

  • Indirect sinus lift
  • Direct sinus lift

The indirect technique is used if the bone height is at least 7-8 mm and the direct one is used if the height is less.

With the indirect techniques a specialized kit like OSSTEM CAS kit is used to condense the bone and lift the sinus membrane and bone graft material is placed into the newly created space.

In the direct sinus lift, a small window is made on the outside of the sinus wall and an opening is created in the bone, which is filled with bone graft.

Dental implants can be placed while performing the sinus lift surgery or after it. If the height of the bone is minimal, it requires from 4 to 6 months for healing and bone regeneration after the sinus lift procedure. The patient should be under regular medical supervision in the postoperative period.

GUIDED DENTAL IMPLANT SURGERY

Advances in digital technologies allow virtual planning and the fabrication of highly accurate surgical guides that facilitate precise implant positioning. This is done via a cone beam computed tomography (CBCT) with a radiographic guide that can later be converted to a surgical guide.

There are some obvious advantages of surgical guides, namely:

  • Flapless approach to placement of implant, therefore less post surgical bleeding and faster healing.
  • Customized guide for each patient.
  • Ideal placement of implant considering available bone.
  • Improved patient confidence in the procedure

Guided implant surgery is often done on patients demand or when the dentist wants to confirm the placement of implant prior to drilling.
